• Required Documents
  1. In addition to a completed application form, each of the following documents is needed as part of the admission process for every student.
  2. Attested Birth Certificate
  3. School Transfer Certificate (if applicable)
  4. Medical Certificate
  5. Two Recent Photographs
  6. Progress Report of the Previous Class
  7. Copies of any achievement certificates – academic, athletic, extracurricular, etc.
  8. Student holding foreign passports are required to submit copies of their immigration documents.
