MANDATORY DOCUMENTS 1. Child’s scanned passport size photograph: - photograph on solid background not older than 3 months. 2. Father’s and Mother’s /Guardian’s scanned passport size photograph: photograph on solid background not older than 3 months. 3. Family photograph (scanned) (Father, Mother, & Child): - father, mother, and child in a single photograph. - photograph on solid background and not older than 3 months. 4. Proof of Residential address (Any of the below): - Voter ID Card (front and back) - Ration card issued in the name of parent (with child’s name) - Passport in the name of any of the parents (both sides) - Electricity bill/MTNL telephone bill/Water bill in the name of any of the parents or child - Please ensure that the document uploaded clearly shows the address as mentioned on the common admission form. - Aadhar Card issued in the name of any of the parents or child 5. Birth Certificate of the child - document should clearly show the date of birth, child name, parent name & signature of the relevant authority. 6. Medical fitness certificate of the child - You can get a medical fitness certificate of the child from a registered medical practitioner. - The fitness certificate must have information about Blood Group, Vaccination, Allergy, etc. - It should clearly show the date of birth, child name and other medical information with the signature of the registered medical practitioner and his/her registration number. 7. Child’s Aadhar Card (scanned) - Aadhaar card (front and back) of the child. 8. Father’s and Mother’s/ Guardian’s Aadhar Card - upload Aadhaar card (front and back) scanned
